Dog Sitting Checklist

In order for us to have all pertinent information while sitting your pet in your home and to be able to go over the details, please complete the below checklist at least three days before your departure and click submit at the end of this page.

NOTE: Please type in the letters ‘NA’ (not applicable) in all the fields that do not apply to you. All fields are required to be filled in. Otherwise the form can not be submitted.

KEY POLICY REMINDER: (this only pertains to people who’s keys we do not already have)

If your building has a twenty-four hour doorman where the keys may may be accessed at any time, we can pick up the keys from the doorman, retain them for the duration of the sitting period and return them on the last day before we leave. However, if that cannot be arranged, it is our policy that we request two full sets of keys – One set for the sitter and one set to be kept at our home office as back up in case of an emergency. Once again, we will return all keys on the last day before we leave.

Other Pertinent Information

Important:
If something goes wrong with the emailed form you filled out and sent to yourself, you can still download the form Dog Sitting Checklist (1651) (WORD doc ) fill it out and then print it. Please put it somewhere easy to find such as on your fridge. Make sure to let us know where you put it.

If you need to change, add, or delete any information after you have submitted the checklist, you need to resubmit the revised checklist form and update your home copy before your departure.

Building Staff and Security:
Please inform the building staff (super, doormen, etc.) or anyone else you feel necessary, of our stay at your home, including our company’s name (Dorothy’s Petcare), our names (Dorothy Pimentel and Darius Ditullio), and the sitter’s name.

Keys:
We request two sets of keys – one for the sitter and one for our home office as backup in case of an emergency. However, if your building has a twenty-four hour doorman with whom the backup set of keys may be left and obtained at any time, we just require one set for the sitter to hold on to for the duration of the sitting period. (Please inform the super and the doormen, etc., that if necessary, we may need to use the backup set of keys at some point.

Payment:
Arrangements and details for payment will be settled before your departure. Cash is always preferred but for those times payment must be made by check, please write the check out to cash or if it’s better for you, you can write it out to ‘Dorothy’s Petcare’.
Please put payment in a sealed envelope marked “For Dorothy’s Petcare,” including your dog(s) name(s) and the dates of the sitting period.

Dog Sitting Cancellation Policy
-35% of the fee will be charged for cancellations made less than one week in advance.
-25% of the fee will be charged for cancellations made less than two weeks in advance.
